Everything we do is tailored to the individual we are serving. It all starts with a Care Plan.
We send a Registered Nurse to your home to perform a complete Individualized Assessment. This appointment typically takes up to 2 hours. The information collected is used to create an individualized Plan of Care that is specific to the needs and choices of the individual as well as the physician’s order.
This includes but is not limited to medical history and physical, medication, skin integrity, mobilitiy, social supports, personal care abilities (bathing, dressing, grooming) needs and financial information (if government funded program available).
The individual and the nurse agree upon a Plan of Care or course of treatment. Care Plans are then sent to the physician for approval at which time a open line of communication is established. We work closely with the physicians which is an important part of treatment as it minimizes hospitalization. The Care Team is assigned and scheduled and Care begins.
